The CMS assigned an overall rating of 2 out of 5 to Broken Bow Health and Rehabilitation, a score below Oklahoma’s state average rating of 2.7 for nursing homes.
Within the same reporting period, Memorial Heights Nursing Center held the largest bed count in the county with 118, while Hill Nursing Home, Inc. was third, providing 51 beds.
By the close of the first quarter of 2026, Broken Bow Health and Rehabilitation had paid $7,443 from a single fine and faced one penalty.

But when you need that type of care, there are limited options.”
